Our mission remains to provide care to all those who are in need. We accomplish this using creative out of the box thinking. We understand that for most of our patients and their caregivers, our paths will cross at one of the most difficult times in our patient’s lives. We understand where you are, and are honored you have allowed us to join your journey.
Supportive and Palliative Care Services
Get additional support for your chronic illness. We provide an additional layer of care in addition to your PCP and Specialists. Our job is not to replace them, but to ensure everyone is talking to each other, and most importantly aligned to the treatment goals you have identified.

Dr. Mohammed Iqbal
Board Certified Physician with focus on Chronic Illness Management and Supportive Care. My focus remains on helping the most vulnerable patients in our health system. The purpose of the my practice and everything we do at Transcend is to provide a place where we can help those who are in need of significant medical assistance. Despite being board certified in numerous specialties (Pediatrics, Internal Medicine and Hospice/Palliative Care), my focus remains on helping those complex patients with numerous medical problems. In my spare time I enjoy spending time with my family, hiking, playing basketball and teaching medical students and physicians in training.
Ameena Hassan, ACNP
I am a Supportive and Chronic Illness Nurse Practitioner and serve as the Director of Clinical Services. I graduated from the University of Texas at Arlington with an ACNP- AG, MSN. I worked as a hospitalist prior to joining the palliative care team here at TIH (Transcend Integrated Healthcare). I have found immense satisfaction working with our chronically ill patients and their families. I enjoy being able to think outside of the box to provide symptom support for my patients. I also enjoy the emotional and spiritual aspect of our job, being able to connect on a higher level with patients and their families. I enjoy working with my colleagues and feel we are all a big family here.
Thompson Ugoh, ACNP
I am Adult Nurse Practitioner with a focus on adult Primary and Palliative Care. I obtained my bachelor’s in nursing from Texas Tech University health science in Lubbock Texas in 2014. I have served as a primary care provider in the community before joining TIH (Transcend Integrated Healthcare). I enjoy using my knowledge to help those in need. In my free time, I enjoy traveling with his family and spending spare time serving my local community.
Fatin Qutob
Specializes in integration of business with the passion of treating the chronically ill. Ms. Qutob has an advanced degree in behavioral science and health from the University of Utah. Healthcare has always been the field she has aspired to excel in. Our programs focuses on quality of life for those facing chronic or terminal illnesses which many others can’t do. Being a person who lives to serve people, Palliative care has become her calling.
Kim Beavers, LVN
I am an experienced hospice nurse. I have always been drawn to help people in their time of need, and have spent years of my life helping those who are terminally ill on hospice. I am proud to be part of the TIH (Transcend Integrated Healthcare) team where I have the opportunity to offer my expertise to patients. I love peace, happiness, making memories, traveling, helping others and quotes. Aaliya Nasreen
I am the Clinical Liaison for Transcend Integrated Healthcare. I’m passionate about spreading awareness through social media, breaking barriers, and informing more people about the exceptional care we offer. Contributing to our patients’ well-being with this compassionate team is incredibly rewarding. Making a positive impact in their lives inspires me daily. I eagerly anticipate continuing my journey in palliative care to help more individuals in need.
Alana Kuykendall
I serve as a medical assistant at Transcend Healthcare. I am grateful to be given the opportunity to help care for our patients and will do everything in my power to help us succeed. My goal is to help as many people as I can. My hobbies include reading, drawing, exercising and gardening.
