Disease Management

We work with your current doctors (Primary Care Provider and Specialists) to help manage your disease process. Our focus is not on treating numbers but getting you to feel better.

OUR FOCUS REMAINS YOU!

We remain focused on treating you as a person, not your labs! Our focus is your entire well being. We provide clarification of plans and goals by providing clear communication with you and your family so no one feels left in the dark.

Constant Fine-Tuning

We fine tune your needs as they change over time.  Your circumstances will change and so will the symptoms we are addressing. We see you regularly so we can adapt our treatment to ensure your well being by focusing on quality of life.

Financial Savings

Our extra layer of support is designed to catch problems early and help keep you out of the hospital and ER. Having physician visits regularly rather than frequent visits to the ER and hospital leads to financial savings for you.

our practice

We provide supportive care for your uncontrolled symptoms to give you an extra layer of support.

Our focus is on pain and symptom management. We understand that sometimes root causes cannot be treated. We allow your specialists to focus on underlying pathology while we focus on symptom control to allow you to live your life with the highest quality possible. We also help with treatment goal planning so all of your healthcare team is on the same page.

We are all emotional and spiritual beings. Focusing on improving quality of life by symptom control, keeping your mind and spirit in a good place leads to a higher quality of life. Higher quality leads to higher quantity – being happier and pain/symptom free will give you the strength to keep going and push through obstacles leading you to living longer.

our practice

We provide top quality highly directed healthcare

A Few Examples of what we do:

  • Person Centered Care – Focusing on you as a whole, not just your individual disease process. We never chase lab values and numbers. Our focus remains on you.
  • Communication – Spending time talking to you and your family. We will call and communicate with your loved ones.
  • Expert Clinician Guidance – Our physicians and nurse practitioners spend time reviewing each care plan with you. All care plans are reviewed periodically to ensure you have the highest quality of care.
  • Symptom Management – We focus on controlling your symptoms. We are the experts that other physicians call when they are having trouble controlling symptoms. It is what we do. These symptoms include: pain, shortness of breath, nausea, vomiting, constipation, diarrhea, itching, confusion, agitation, edema, and so much more.
  • Coordination of Care – We coordinate care between all of your providers including physicians, consultants, home health services, and health care facilities.

We help find the proper treatment for every patient

Given the level of complexity of our patients, we have significant experience with alternative treatment options, sometimes including those not thought of by other physicians.

Sometimes, there truly is nothing more that can be done, and you need someone to have a difficult conversation to come up with a plan that aligns everyone’s goals. We don’t shy away from having these care plan meetings. Our focus remains on giving our patients the highest quality of life, no matter what we have to do to achieve it.
